ASSUMPTION OF RISK: I understand and acknowledge that participation in martial arts activities, board breaking, Nerf Wars, and other physical activities at Awesome Birthday Parties / Warrior Martial Arts ("the Facility") involves inherent risks, including but not limited to: physical injury, bruising, sprains, fractures, and in extreme cases, serious injury or death. I voluntarily assume all such risks on behalf of myself and the minor participants listed above.
RELEASE OF LIABILITY: In consideration of being permitted to participate in activities at the Facility, I, on behalf of myself, my child(ren), heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Awesome Birthday Parties, Warrior Martial Arts, their owners, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained while participating in any activities at the Facility.
INDEMNIFICATION: I agree to indemnify and hold harmless the Facility and its representatives from any loss, liability, damage, or cost they may incur due to my participation or the participation of the minor(s) listed above in the Facility's activities.
MEDICAL AUTHORIZATION: In the event of an emergency, I authorize the Facility staff to seek medical treatment for any participant listed above. I understand I am responsible for all medical costs incurred.
RULES & CONDUCT: I agree that all participants will follow the safety rules and instructions given by Facility staff at all times. I understand that failure to comply may result in removal from the party without refund.
GOVERNING LAW: This agreement shall be governed by the laws of the State of New Jersey. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
